How do i set a form up to publish the results on my site automaticly?

i have a form here..

its published.. but i get an error when submitting

id like the results to be shown on a seperate page.. something like

http://www.goodfellasguild.com/guildfiles/raidmembers.htm

can this be done without me having to manualy edit an html page?

this form will be used to sort out raid groups in my guild for world of warcraft
