How do I populate a listbox from SQL??

OK, forgive my ignorance! My companies VBA person left and her work load was given to me (someone who has no VBA training or experience).
I need to update an exisiting Macro. It's a Macro that creates a fax. I need to add a ListBox of codes (these codes will need to be pulled from SQL) to the Dialogue Box.
Please help!
BTW - We are still using NT as our platform.
