How do I make a form?

I'm pretty new to Notes. I'm trying to send an email that will allow recipients to vote for one person from a list and have that selection emailed back to me. In HTML, I could write a form with radio buttons or use a SELECT list and use JavaScript to check to make sure a selection was made. Is there a way to do this in Notes?

For anyone who finds this thread through a search it turns out that you need Domino Designer to create survey form.
