How do I install a "vote" option?

I need to use a simple yes/no vote on my site, which once voted will show the results so far online.
I use DW3, and my host has CGI script ability.
Please advise, and keep it simple!
Also how would I reset the vote, and would it be possible to stop someone voting more than once? [sig][/sig]
