how do i find the model and mfg?

I have an internal modem. I have a P1 and running WIN98. I need to install the driver. I do not know the mfg or model number. I have done some searching with engines and on us robitics and hayes and have not found anything. On the card there is a "FC" and then pc156019 and pm560lh. I think it may be a Hayes, but every search led to a dead end.
