How do i add a subscribe option to an HTML newsletter?

Help!
I want to add a subscribe/unsubscribe option to a HTML newsletter i will be authoring. I have produced a form using HTML to add the the newsletter but i am having problems finding a suitable javascript and writing my own seems daunting. Can anyone help?

any subscribing would need to be done with server-side code and a database. i suggest asking this question in the appropriate server-side forum (perhaps php: forum434).
