How do Dial Phone and bring up Note Entry

I would like to use the Dial phone feature in FM 7.0 and have a call note field/dialog/layout popup at the time the phone number is dialed. A date, time and a call note would be entered. When the note is completed the call would be disconnected. Any script ideas would be greatly appreciated. Thanks!

Start your script to dial the number, make some statements in case there is mo answer, line busy etc.
Activate, within this script another script for your notes, return to the previous script to close this script when the call is finished.
