How are these IPhones connecting to secured wireless AP's

We have two different wireless setups each at different locations. At one location we have Cisco Aironet 1200 series AP's secured using WPA2 and AES. At the other loaction we have some old Enterasys AP's using MAC filtering in AD. At both locations there are people freely connecting to the access points and pulling an IP address via DHCP. I want to know if anyone else has had or is having the same issues and what was done/being done to resolve it?
 
If these AP's are truly secured using WPA2 Authentication and AES Encryption then the only way these users are connecting to them is if they know the WPA2 PSK - assuming you are using Pre-Shared-Keys as opposed to EAP & centralised Radius authentication?
It is possible there are other (rogue?) AP's at the site? Have you verified on the AP's what devices are associated when you think these iPhone users are connected?
