This happened on my home system and was just wondering what it could've been.

I happened to check on the thing one day and the first line light on the mlx20l was green, but speakerphone was off and handset on hook. VMS R3 was just sitting with no calls. I tried dialing out with another set and got a fast busy, no lines available (there's only one line).

I flicked lines around on the 20l and the light went out, and then I could call out.

Is this what happens when an inhouse set is done with a call but doesn't hang up? The line is groundstart, i didn't think it would stay "up" like that. Everything works fine now, but I keep wondering what it was.
 
It is indeed what will happen if a ground start line is reversed. Although polarity is not a problem with loop start lines, it is imperative with ground start. The fact that you are just now experienceing this leads me to think someone accidently disconnected your line or saw a jumper or splice reversed and then 'fixed' it.
 
that's interesting, and very maybe what happened!

I bet this is the only groundstart in this prefix so it it probably gets 'special' attention.
