Help with check boxes 1

I'm using Acrobat 4.0 to create forms that users can fill in and print using Acrobat's form tool. I need to create two columns of check boxes where only one box on each line can be checked at any time ("Check YES or NO&quot. I've tried using check boxes and radio buttons, but can't seem to get it to work correctly...can somebody give me a hand on how to do this correctly? Thanks!

Oops...forgot to mention that if the user checks "YES" I want the "NO" box to uncheck itself automatically and vice versa.

Make sure you select radio buttons for both.

Both of the buttons fields have to have the same name,
but make sure they have different export values
