I have a website running on a win 2003 server std. In this website there are a couple of submission forms that ask for a username and password when the user clicks on submit. It did not always do this. Can someone please help me. Below is a link to one of the submission forms. When you click submit you'll see what I mean. Please, any help would be MUCH APPRECIATED!
