Help!!!make a query from the source table

I have a table named "source", "Source" has three field,"HTH", "XH" and "CO".
HTH XH CO
1. 20 1 H2
2. 20 6 H2
3. 20 1 N2
4. 20 1 O2
5. 60 1 H2
. . . .
. . . .
. . . .
I want a result like this:

FieldName:HTH XH XX(New field)
20 1 H2 N2 O2...
make a form or report: list according "HTH" and "XH" , and list all componet in a new field.
