Help accessing SQL data from external pcs via API

SQL Server 2005:

Scenario:

I have created three databases containing patient data/info and am tasked with providing access from external pcs. The key requirement is the need for end-users to be able to access/add data to these databases from a simple front end; i.e. API or webform. Has anyone had any experience with what to use to accomplish this? Suggestions? Ideas?

That's a very broad description of the requirements. In any case, you'll need to program (or find/buy) an application to implement your business rules and interact with the database. What programming languages do you know?

I assume the information in the three databases is related. Is there any particular reason you decided to create three databases instead of just one?
