Having issue with lto3/hba on a v440

I'm having a issue with one of my LTO3 tape drives on a v440:

v440: 16gigram, 4cpu, 3lto3, 2dualhba

Everytime Netbackup attempts to use one of the tape drives (st15) the hba goes offline then back online...

Please note: Netbackup can see all drives & serial numbers..

Note sure if this issue is a hardware one or a config one...

Joe, it may or may not be relevant but I have experienced issues with certain SUN models and Qlogic adapters under Windows (galaxy series) that sound similar to what you are experiencing. I would try moving the HBAs to the PCI slots that support higher speeds (at least move the one in slot 2). See if that helps. Check with SUN support regarding this.
 
I figured there are several things I could do:

1: reseat the card in pci4...
2: Roll the fiber going to the tape drive to the empty port..
3: Roll the hba from PCI4 to the empty 66mhz slot...
