I have received similar errors. Usually, I also get a separate error about the physical disk the LV is on. Have you gotten an error message about a disk having problmes yet?

If you have IBM support, you can call them and ask them about codes (the sense data) that came out in the error report.

I suppose the can be caused by a number of things, but the bottom line is you most likely have a problem with the physical disk (or disks) the logical volume is on. I have gotten this message when I lost two logical volumes on a disk that had gone bad. LVM just couldn't find them anymore and reported an error.

I have also received similar errors when nothing turned out to be wrong with the disk or any of the logical volumes on the disk. I did have to sync up a mirrored logical volume after receiving a similar error, but there was nothing wrong with the disk.

If you don't have the logical volumes on this mirrored (or RAID5 protected), do everything you can to get the data protected as soon as possible.

You should run the diag (if you have it available on your system) command against the disk that seems to be giving you problems. For a full diag against the disk, you have to take the disk off line -- either by getting all the LVs on ths disk off to another disk or by bringing the entire system down to maintenance level.
