Hard Drive Noise on Read/Write Access

The hard drive makes a distinct noise, like a "crunching or scratching noise" when accessing the hard drive for any operation, i.e. writing or reading from it. Is this an indicated the drive has a problem even though it still functions ok? When there is no activity to the HD there is also no noise from from it.
