Hackers in Voicemail

We are experiencing hackers in the voicemail systems. They are setting up remote forwarding to other countries and causing some serious phone bills with customers. Some of these folks use remote notification so we can't deny line pool access to the mailboxes. Any suggestions.

 
search the forum- there have been a few threads on this.
-Change all passwords- don’t let them be easy
-delete any unused mbx's
-remove outdial from those who don’t need it
-remove international at the CO if they don’t need it, or have the CO put on account codes
-restrict link capability at the system and CO if not needed
 
make user use a 6 to 8 digit password and restrict international calling with filters
