We have a customer with TDA30 and tvm50 and some how there long distance has been hacked ( long distance calls being made at night thru there system) be have made sure they have strong voicemail passwords and i dont see any sets with cll fwd to CO enabled - Any ideas on how this is being done?

I'm not sure about your dialing plan, but anything beginning with 9xx is fair game to assign an outside line and wait for additional digits. The voice mail doesn't know it's an access code, it just blindly dials it. Try restricting the port the VM is connected to with a Class Of Service. Restricting outward dialing usually stops it.

Mostly they allow call fwd through mailbox and have being known to go in at night and set it up and trur it off again in the morning
So put all extensions assigned to vm to cos 7
Check in cos if mailbox that call fwd through mailbox is not allowed
Strong passwords on 999/998 or no phone access, as they can do what they want if they can access these
Make sure they have not set up external message notification in un-used mailboxes
