Groups in Charts

I have a report which has a count on the group. Now I need to put that count into a chart but haven't worked with charts.

Group: Status_code (I currently have 5 active and 10 completed)
this is the only group I have.
Would like the line chart to show:

10 9 1
(line chart) (line chart) (line chart)
Completed 2012 completion 2013 completion

My record selection has me getting the last 2 years and two years in the future.

Currently in the chart, it does show each year with the line chart but I need to
1. restrict that to 3 columns
2. get the counts to show above each column
3. Change the axis label to be completed, 1 year out completed and 2 years out completion

Please explain your exact setup in the chart expert and also identify the columns that are currently being returned. Also, what are the current axis labels returning?
