Grouping

I am building a form. My table that I used for the form consist of monthly feilds from Jan. all the way to the end of the year which of course would be Dec. I am trying to group these feild together so that if the user wants to choose a certain month they can. Can someone help me out, with this grouping on my form?
 
What do you mean by grouping? Are you speaking of them being in a certain position on the form?

I was thinking more of one box in each month and select what I want.
 
You need to look at the Continuous Form option in the Default View property of your form (Format Section)
