Trying to get a group call to go to voice mail after a certain amount of rings. I have it working properly if the users put their phones on DND, but if they don't do anything, they just ring and ring. Here's the config:

What am I missing?

Try it with:
Queue Control Limit 99
Overflow Threshold 1
Overflow Threshold time 0
and see what happens.

Also, I have found that when changing calling group programming, sometimes the changes don't fully take effect reliably unless I cycle power to the phone system.

Also, I believe that a circular queue set to auto login will never fail over to a coverage group unless you program ES2 buttons or DND buttons. Since you set all extensions to auto login, the system thinks all the extensions are available. It will just keep bouncing the call back and forth between the numbers. If you don't want them to be doing a login/logout, available/unavailable, or DND, then you could just program a button on each phone to cover the other extension and not use groups at all. This way both phones can ring at the same time or delayed when either extension's number is called.

Well, Danny28 is almost right.

Part of your issue is this:

"Overflow Threshold (#) : 2"

That means, it will not overflow until you have 2 calls in queue.

-AND- the callers have waited 21 seconds...

To make it just be 21 Seconds (WHICH IS A LONG TIME, kinda' like 5 rings) Set:

Overflow Threshold # to 99.

I don't know why, but at this part of the programming, 99 = ZERO.

Let us know how that works for you.
