The 4.1 Upgrade Guide on RIM's web-site has most things you need to know.
Why did the upgrade to 3.6.x fail?
What version are you currently running (need to be at a minimum of 3.6.3 to upgrade to 4.1)?
Do you have multiple instances running on the 3.6.x BES?
What versions of HH code are you running?
