ghost tapes seen in drive

a couple of times within the past month or so, our backup exec has been seeing ghost tapes in our drive. for example
we have drive1 and drive2.......our backups would be directed to whichever drive of course......but what happened
is that drive2 will be in the pending state showing the media label and all that, but there is physically no tape in drive2.........how is that possible?

I have seen something similar with a HP Ultrium LTO library, it was caused by a SCSI terminator problem. Are you running a library? If so, what happens if you initialise it from within BE?
