General Website Data Entry VFP Form

I would like to develop a data entry form (it would be several pages) that would allow online data entry by individual facilities. The form would be located on our agency's web server.

This data could then be downloaded to our server.

Does anyone have any pointers here, or can point me in the right direction to do the research I need?

I just want to second Mike's suggestion to head to west wind. They have a great example program using fox front UI end over the web and a LOT of support material.
