FrontPage 2003 form submit button broken

I have a form to collect info for a mailing list. After the person enters the data and clicks Submit it first appears that nothing has happen. Then the person gets an error message. The error is an error as the data has been successfully sent. Because of this people are clicking on the Submit button repeatedly thinking it is not working. The site is hosted on a server that has activated FP extensions. I am hoping someone can look and see where the problem is.

Does _vti_bin/shtml.dll/mailinglistNew.htm exist on your server? From the naming, it looks like some internal FP directory. Should the code be going there or to somewhere one level up?
