Are there limitations on the "from" field when you receive a notification of an email w/the wav file? Currently our information will display the extension only on internal calls...is there a way to expand that to include the caller's name and if so, where is that controlled?

unfortunately MM will only know about its own subscribers (and networked subscribers in MM for MSS systems that are networked).
The reason you are getting that from a call that was internal is either the number does not match a MM subscriber on that system or MM does not have the ability to Send As the MM subscriber. Are you using MM for Exchange?

What storage backend are you using?
Are you using Notify or Call Me function through Subscriber Options?
Are you using any clients (Web or Email)?
Are internal calls routing through the MM first?
