I know with previous versions of Exchange, you could run isinteg and eseutil manually on the database to free up space after deleting mailboxes or messages with large attachments, is this still the case with Exchange 2003? Must the utilities be run manually or does the mailbox cleanup wizard accomplish this?

If by "free up space" you mean decreasing the size of the information store then yes, you need to manually perform the offline defrag. Exchange 2003 performs online defrag nightly but the size of the db does not decrease. The offline defrag rebuilds the store without any white space.
