Free Feedback form

I want to add a feedback form to my website
I know some websites provide free guestbook or feedback pages. Does anybody know of such wewbsites? (needs to be free). My intention is to have a link from my website to this other webpage where guests can leave comments.

What language do you use? Can you have a database? Building your own would probably be the best.
