Forwarding issues

My customer has an PACS R7.0 91E with a 308 EC and a large pc card voicemail in a 5 slot carrier. They have 8 POTS lines programmed onto the phones. Three phones are currently using the forward feature when the user is out of the office. One out of the three is forwarded 95% of the time. Recently I was asked to forward ext. 10 to their NY office. When ext. 10 has the forward feature activated; the ext. that is forwarded 95% of the time gets a fast busy when a call is made to that ext.
So, Basically, a call comes in, AA picks up, you dial the ext (in this case ext 17) AA transfers the call, then you hear a fast busy. This is happening when all 8 lines are free. When I unforward ext 10, everything works the way it should.
Any insight to this would be appreciated. I've tried all the troubleshooting I can think of except for programming the whole switch from scratch.

Can you replicate while on site? Is fast busy coming from telco? You MAY need a pause as first digit of outgoing number in order for telco to collect all of the digits properly.

try to make sure that the Auto LIne select selects all lines and not only a few of the 8 lines you have

Mforrence- I haven't been back to the site yet. But, when I call there and get transferred to ext 17 I get the fast busy. When the ext 10 forward is removed I don't get a fast busy. I'll try the pause next time I go there.
Westi- I'll try your suggestion also. Never thought I would need to do that.
