Forward Unconditional issue

R9.1.100.10 Virtual Linux SE and IP500v2 Expansion for PRIs. Our primary inbound calls come in from the PRI (IP500v2 Exp), primary outbound on SIP trunk on SE. When I set up Fwd Unconditional on and ext it ends up in user's vm box. I've done some testing and it seem it only fails when the call comes in on PRI and tried to forward out on SIP.
Here are some different scenarios I've tested:
Internal extension to Forwarded Ext, out on SIP, OK
Outside call inbound on PRI to Forwarded Ext, out on PRI, OK.
Outside call inbound on SIP to Forwarded ext, out on SIP, OK.
Outside call inbound on PRI to Forwarded Ext, out on SIP, NOT OK, goes to vm box.

I have Inhibit Off-switch Forward/Transfer unchecked on both systems.
Any assistance is appreciated.
 
Do a monitor trace with SIP TX/RX only, sounds like it might get denied by the SIP provider since IPO sends incoming CLI as in the from header.
