Forward All turns on mysteriously for 7941 IP telephone

We have Cisco Unified CM version 9.1.1.20000-5. One particular 7941 telephone will just start call forwarding all calls to voicemail. No rhyme or reason and it occurs @ every few weeks. The set in CM shows CF off. We reset the phone and all back to normal. Anyone heard of this one?
 
Can the phone receive and make calls when that happens?
 
The user can make calls, but incoming calls all forward to VM as if the feature is active on the telephone.
 
you said the set shows CF off. What does the call manager administration page show for the line forwarding?
Have you tried replacing the phone to eliminate a faulty set?
 
CM shows it turned off too. I have not replaced the set, but was planning to tomorrow.
 
You could try to change the soft key template for the phone to remove the CfwdAll. Then see if it's still forwarding after that.
