Forms within Director

I'm up against a problem,

I have been working on this Multi lingual Director project and have nearly completed it, all that I am stuck on is implementing a form based email page, it consists of selectable tick boxes also name, address and telephone fields, what i need to happen is for the information to be collected and sent off to an email address.

is this possible
