forms that refresh after option select

Hello all. I'm trying to make an online RSVP form for my wedding next year, and we want the form to display specific options on refresh based on some choice they make.

For example, the question: "Are you attending?" User selects "No", then the page refreshes to display a text area labelled "Note to the Happy couple" (I call it the "excuses for not attending" box).

Any help or links to a site that shows how to do this with PHP (the only dynamic server-side language I have any familiarity with) would be appreciated.

I suggest a client-side scripting language like Javascript.

If the question is strictly yes/no, I recommend using radio buttons, and using the onClick event.
