forms on acrobat

Hello,
1) I have adobe 7.0 Standard _ did this allow me to make forms or must I get profesisonal

2) Is there a way to create forms so that they can be posted and completed online (possible converts pdf form to HTML form). For my medical practice, I have patients competleting the reigstration form both online ,or via emailed pdfs forms thy can complete.

...these links cover the topic pretty thoroughly, adobe haven't made form creation as straight forward as one might think, there are hoops to jump through with regards to how the end user can save a pdf form using the free acrobat reader:

...adobe livecycle is often used to create forms (templates), however it is windows only, not for mac, although there possible workarounds that have been documented:
