Forms in Outlook 2000

I confused as to how to use/design forms in Outlook 2000.

What I want is some sort of email templates, with some 'blanks' which are determined by what is filled in the forms.

What I did manage to do is
1) Compose a new mail,
2) Type in the template,
3) Save it as template format
4) Open the template for design.
5) Add 2 user defined fields.
6) Add text-boxes corresponding to 5)

What I don't know is to how to relate the content of 6), so that they get automatically inserted into 'blanks' in the email templates.

Note, when I open email template for design at 4), I could see what I saved in 3) at the Message Tab of the 'Edit Compose Page', and also the texboxes I added.

Any ideas??
