Forms don't show up in web browser

Please forgive my ignorance. I'm been searching for a while on a solution, but can't seem to find an answer. My problem is whenever I want to run a form, my browser comes up but does not show anything. I did download the jinitializer when prompted, but that didn't help. Now I just get the name "ORACLE FORMS" in the top left corner of the firefox browser screen. Is there any configuration settings I need to check. Is there anything I should look for when running the OC4J instance. I also see all this garbage after the localmachine and portnumber in the url bar after running the form. This doesn't match the application server url in the preferences.
