Forms - Conditional Submit or multiple submit buttons

Hello,
I am trying to get a form to submit to one of 10 email addresses. I have tried to use radio buttons to select the mailto, everything was looking great but I never got the email in my inbox. I still think the prob. is in my code. Does anyone know about using maultiple submit form buttons?
