form to make new table

Is it possible. I have a table in which. I have a system and a inbound system and a series of questions.

What I want to do:
When Entering the system name and the inbound sytem on the form I want a new table to be created tracking only this info so i can run a report on it. Thanks for the help.

Take a look at running a MakeTable query from a command button on your form.
