Form to make new table

Is it possible. I have a table in which. I have a system and a inbound system and a series of questions.

What I want to do:
When Entering the system name and the inbound sytem on the form I want a new table to be created tracking only this info so i can run a report on it. Thanks for the help.

My question would be, why do you need to create a separate table just for reporting purposes? Can't you execute the report from the form, on that record, or base it on a query?

Can you explain more?
