Hi, I got this request, and I am not sure how to proceed. We want to be able to create a form that would look something like this:


It would have drop downs that would have to get their data from somewhere ( 2 are pretty static but one would update often)

This form needs to be able to be emailed. Basically we would keep this form, or a link to it, on our website. Then any time any user wants it, they go to it, fill it out, and click send. The recipient is always the same.

I am looking for any suggestions as to how to best accomplish this.

try formmail. i have not used it in awhile but from what i remember it will do what you want.

it is a freeware program, just google it and try it out
 
They closed it down due to spammers and now you have to pay.

We were trying to use word, but it asks the user if they want to allow a program to be allowed to access the exchange server when emailing the form. We don't want that.

We just want them to fill out the form, in whatever program, outlook, word, excel, asp? (not best at creating that!) and hit submit and have it email the form to where it needs to go, all filled out.

Any other suggestions out there?

does where ever you are hosting the pages support PHP?

if it does, it would require the use of three files.
1. your form page (html)
2. a php form file
3. a congrats, you sent your request page(html)

Can you explain one thing to me? What is the php you talked about? What do I need for it to work, and how do I work with it?

well... from what I understand.

PHP is a general purpose scripting language used to create dynamic, database-driven websites. It provides built-in support for the popular MySQL database format.

What can PHP do for you? It can create dynamic pages that are not only exciting but useful as well. A popular use for PHP is creating discussion forums. Other uses include guestbooks, calendars, and complete content management systems.

you need a ISP or server running PHP software(which enables you to use php files.) you can download php from if you are running your own server for hosting websites) otherwise ask your ISP or where you are hosting if they support php. if they do GREAT!
if not...ask them why. I could recommend some hosting companies if you'd like.
I'm still new to php...but I know enough to be dangerous.

I use the php only for forms to send email with information entered, but php is much more powerful than that.
