form talk-submit job applications

I am able to write a PHP script that obtains form information and writes it to MYSQL table and assemble email. I am also able to redisplay from table to browser. What I am unclear of is when the form is a job application and you obtain the
employee info, you write it to table but what is the best way to format the obtained info for use by the H/R dept. Can I get some ideas from anyone?

This is really an application-specific question; I would ask HR in what format they would prefer the info and then do it the way they want.
