try putting this in the body tag

leftmargin="0" topmargin="0" marginwidth="0" marginheight="0"

this should remove any margins that upset the printer.

or you could set up the item you want to print as a PDF document. There are several free progs out there to do this.

I prefer the PDF route as you have more control of the final output

I agree. I think that putting a link to a PDF version of your form is a good way to go.

Having said that, I tried printing your page and found the form fitted on, so different users may have different experiences, based on the way they've got their browsers set up. I use IE6 and have the page setup margins to 11.68mm (the smallest it'll go) for left/right margins and 10mm for top and bottom.
