Form problems

I'm using a PHP form (Novice Form, freely available on the web) which is very simple. For me and everyone I have asked to check it works OK, it does...with one exception. Using the same connection, someone has tried it on 1 Linux and 2 Windows machines. In all instances neither I of the other intented recipient has received the email which the form generates. The person who completed the email received no error message and did receive the resultant screen confirmation that the form had been sent.

I can't work out where to begin on this...any ideas?
