Form no longer finds the correct inside address

I have had to modify someone else's form (they are no longer employed by our firm) for a logo change. Now, when the end user executes the Format, Envelope feature in WordPerfect 9, WP finds the address in the tabled letterhead area of the document.

Previously the letterhead was also in a table. Was it protected somehow? I can't find why the change between the two forms.

Would someone have a suggestion for me?
