Form looks wrong in Firefox 1

Dear Web Gurus,

I have created a one page website for a pawnbroker:-

http://www.newbondstreetpawnbrokers.com/leabridge/

The contact form looks fine in IE, however in FF the "name" label and field are not aligned.
