Form in outlook 2003

Hi,

I am trying to create a form in outlook and there vbscript inside the form. I did the publish but when i use and send the form,the form format still email message not a form so my vbscript cannot run

does the form need to publish in organizational form library in my exchange server

I am using outlook 2003

Any suggestions will be useful
