Form Creation Programs

Hi folks.

I am looking for some sort of Form Creation program that will let me design ASP type forms quickly.

Users currently complete a Word Document with Form Fields. They then save it and send it to me as an attachment. While this works to a degree there is a lot of admin overheads.

So I would like to design an online form using ASP for example. The data would then be stored in a database/emailed to me copied to user etc etc.

Anyone know of such a program?
