First Name Last Name fields

I want to initially ask the person who is filling out the form, "What is your name?" and continue that format for other information, such as social security number, phone number, etc. Well, on some of the forms it asks specifically for both the Last Name and for First Name, not just one field for the full name. Then, I have a few forms that start out like this, "I, (insert name), certify that..." and if I put the Last Name and First Name fields next to each other, there is a huge space between the two, depending on how long the person's name is. Is there a way to truncate the spaces, and how do I tell it to do this?
