I need to fill out a spread sheet for an upgrade we are going through and need help finding the info to make this:

Extension/Circuit *
Port/Segment Number
Telephone/Circuit Type
Organization
Billing Account
Site *
Building Number *
Floor Room Jack
Personnel/User
Misc Information

Site *
Bulding Number *
Building Name
Building Type
Terminal Terminal Type
Cable *
Cable Type
Starting Pair *
Ending Pair *


Most of that I can fill in, but I wasn't sure if there was a report I can just have printed out. We had one from years ago but no one has any idea where it came from. Thanks Y'all

Guess what I mostly need help with is that if there is a way I can find out where I can pull this info:

I don't think the loads Eddie gave you will help. I think they are wanting PRI circuit information? What are you trying to upgrade?
 
We are getting an upgrade to a TMS and have to install a Telebox unit then we are scheduled to have a remote install done with the contractors for this upgrade. The sheet I received is asking for all this info to be filled out that I need to find and being I'm newer and took this position over without much notification, I'm in learn mode!! I truly appreciate your all response and help with this as I progress here, hope the lingo isn't confusing.
 
I'd be starting with your phone bills. A "billing equipment record" or a "customer service record". What your system has programmed is certainly no true indication of what you actually pay for. First find out what you pay for, then find out where it connects at your site - some lines with be the phone system, and many will lines nobody even knew existed.

I got all the info I needed, Thanks Everyone. Eddie those codes gave me everything I needed and I was able to get the rest the info through our records! I truly appreciate it!
