Fillable pdf - checkboxes

I'm a complete newbie to Adobe Illustrator/Adobe Pro so hopefully this isn't too stupid of a question.

I had an existing pdf that I needed to edit. I opened it up in Adobe Illustrator and made the changes. When I opened it back up in Adobe Pro, I saw that I had lost the ability of a fillable pdf. So I added all the fields back to make it fillable.

My problem is that when I added the checkbox fields, I end up with the outline of the checkbox field plus the outline of the existing box. How do I fix this?

Like I said, complete newbie question but I can't figure it out. Thanks!
 
Don(t use Illustrator to create a form.

Use Acrobat Prfessional.

Make the layout in any program than use the form options in Acrobat Professional to ad your checkboxes and so on.

Or if you work on a windows machine use Adobe Livecycle designer. This program is specially made for creating forms.
